If we’ve worked together previously, welcome back! I am so happy you’re here! Eligible returning patients may schedule a Continuity of Care Visit for $99 through December 31, 2026.

This fee covers the visit only. Medications, including GLP-1s, labs, and other services are separate.

Testosterone therapy is individualized to your symptoms, goals, and clinical needs. Medication is compounded by a specialty pharmacy and is not included in the visit fee. Cost varies by prescribed dose.

Follow-ups are required every 3 months. Periodic labs are required to monitor testosterone levels, guide dosing, and support safe treatment. Lab costs are separate from the visit fee.

 Rooted Wellness Services is a self-pay practice and does not bill insurance for visits. When appropriate, prescriptions can be sent to your preferred pharmacy and laboratory testing can be ordered through facilities that participate with your insurance. Medication, laboratory, and pharmacy costs are separate from your visit fee.
